This policy explains how Texas Functional Health Centers (“we,” “us,” or “the practice”) handles information collected through this website.
This is not our HIPAA notice
Protected health information — your medical records, treatment history, and anything else related to the care we provide — is governed by our Notice of Privacy Practices, not by this policy. If you are a patient asking about your health information, that document is the one that applies.
This policy covers the ordinary website information described below. Please do not include detailed medical information in website forms or email; neither is a secure channel. To discuss your health, call us at (469) 334-0624 or use the patient portal.
What we collect
Information you give us. When you submit a form on this site — a contact request, an appointment inquiry, a newsletter signup, or the Movement Age assessment — we receive what you type into it. That typically means your name, email address, phone number, and whatever you choose to write in a message field.
Information collected automatically. Like most websites, ours records technical details such as your IP address, browser and device type, the pages you view, how you arrived, and how long you stay. This is collected in aggregate to understand how the site is used.
We do not require an account and we do not ask for Social Security numbers, insurance identifiers, or payment card details through this website.
How we use it
- To respond to your question, request, or appointment inquiry
- To send you the results of an assessment you completed
- To send practice news, offers, or health content, if you asked to receive them
- To understand how the website performs and improve it
- To meet legal, regulatory, and professional obligations
What we do not do
We want to be unambiguous about this, because privacy policies often are not:
- We do not sell your personal information.
- We do not rent, lease, or trade it.
- We do not provide it to third parties for their own marketing.
We share information only with vendors who help us operate the practice and the website, and only to the extent they need it to perform that work. Where a vendor may handle protected health information, we require a Business Associate Agreement as HIPAA requires. Beyond that, we disclose information only when the law requires it, or when it is necessary to protect the safety of a person.
Cookies, analytics, and advertising technologies
We use cookies and similar technologies to keep the site working properly and to measure how it is used. This may include analytics services and advertising tools operated by third parties, which set their own cookies and receive information about your visit — including the pages you viewed on this site.
You can set your browser to refuse cookies or to alert you when one is set. Some parts of the site may not function normally if you do. You can also opt out of many advertising cookies through your browser settings or through industry opt-out tools.
Third-party services we link to
Some actions take you to services operated by other companies, each with its own privacy policy and terms:

- Prompt EHR — existing patient scheduling and patient portal
- Cherry and CareCredit — patient financing applications
- Stripe — online payments, where offered
Once you leave this site, the information you provide is handled under that company’s policy, not ours. We encourage you to read them.
Email, calls, and text messages
If you give us your phone number, we may contact you about appointments, care, and practice matters by phone or text. Message and data rates may apply. You can opt out of text messages by replying STOP, and out of marketing email using the unsubscribe link in any such message. Opting out of marketing does not stop essential communications about your care or appointments.
Your choices and rights
You may ask us to correct information we hold about you, ask us to stop sending marketing communications, or ask what website information we have. Write to info@texasfhc.com and we will respond promptly.
If you are a patient and your request concerns your medical records, HIPAA gives you a right of access to your health information, and any fee is limited to what HIPAA permits. Those requests are handled under our Notice of Privacy Practices — please contact our Privacy Officer, Adam Bohnenblust, D.C., at (469) 334-0624.
Texas residents. Texas law provides certain rights regarding personal data. Many of these do not apply to information already covered by HIPAA, but if you believe you have a request under state law, contact us at the address above and we will address it.
Children
This website is intended for adults. We do not knowingly collect personal information online from children under 13. If you believe a child has submitted information through this site, please contact us and we will delete it. Care provided to minor patients is handled under our Notice of Privacy Practices with the involvement of a parent or legal guardian.
Security
We maintain physical, electronic, and administrative safeguards designed to protect the information we collect. No website or method of transmission is completely secure, however, and we cannot guarantee absolute security — which is why we ask you not to send sensitive medical details through website forms or ordinary email.
Links to other websites
Our site may link to websites we do not control. We are not responsible for their content or privacy practices, and this policy does not apply to them.
Changes to this policy
We may update this policy from time to time. When we do, we will revise the “last updated” date at the top of this page. Material changes will be made clear on this page.
